Our office will use WaveLink Forms throughout the year to gather important information from chapters. Below you will find current forms our chapters should be familiar with and utilizing. Students must be logged into WaveLink in order to access and submit the forms.
Event Notification
What constitutes an event: Any activity or event sponsored or endorsed by the organization, including those that occur on or off organizational/university premises.
As outlined in our Events Policy, chapters must submit an Event Notification when any of the following criteria apply to their events:
- Alcohol will be at the event
- Contracts are secured for the event
- Physical activity (e.g., trampoline parks, tournaments, etc.) is part of the event
- Event is overnight
- Event involves guests/non-members (e.g., step show, philanthropy, Family Weekends, Tournaments)
Deadline to Notify: Three business days (Monday - Friday) prior to the start time of the event.

New Member Roster Update
When a chapter takes new members, they are responsible for maintaining an accurate new member roster with FSL. Chapters should submit any changes to their new member roster (adding new members or removing new members) through the New Member Roster Update Form.

Officer Update Form
Immediately following officer transitions, chapters should submit an Officer Update Form via WaveLink. This is to ensure that the staff and councils are communicating with the correct officers. Before you begin filling out the form, make sure you have the name, phone number and email address for every officer.
Keep in mind, the titles that we use are generic and may not exactly match the titles within your organization. Please supply the officer information that best fits the requested information.
OFFICER UPDATE
Membership Status Change Form
Chapters should utilize this form to let us know when members of your chapter are no longer active due to withdraw, transfer, or disaffiliation mid-semester. This form should NOT be used to submit initial and final roster updates but rather to share real-time updates regarding individual members. Graduating members will be removed from the roster at the end the term through the final roster update process.
