If you have been asked to submit additional information for Federal Verification, please be sure to check SeaNet under Student Requirements and Active Messages. Please Do Not Submit Documents Until You Are Selected For Verification. Completed and signed forms should be faxed, or scanned and emailed using the following contact:

Due to the redacted nature of IRS documentation, Tax Return, Wage and Income and Verification of Non-Filing documents requested from the IRS must be sent to you directly, and then a copy should be submitted to our office. Student ID numbers are required on ALL documents. Documents submitted without student ID numbers may not be accounted for properly.

You may mail, email or fax required documents to our office. If you choose to email, please redact or otherwise obscure sensitive data (SSNs) while ensuring that the student's name and ID number remain visible on each page.

You are not required to use the DocuSign/E-Signature option that is offered on some of the forms below. If you prefer to print and provide a handwritten signature, you are more than welcome to submit the documents in that format instead. You may deliver this signed document in person; scan and email; or fax to 910.962.3851.
