UNCW works very hard to make sure that our campus remains as free from hazards as possible and that our campus community is a safe place to live, work and learn.
All incidents and/or near misses should be reported to Human Resources (HR) and Environmental Health & Safety (EH&S) to help promote a safe and healthful campus community. An unsafe condition can be confidentially reported to EH&S without fear of retribution or breech of confidentiality.

- Supervisor's Injury Data Report
- Supervisor completes/signs DocuSign form, and copy automatically sent to HR Benefits Counselor
- Witness Statement Form
- Witness completes/signs DocuSign form, and copy automatically sent to HR Benefits Counselor
- Refusing Medical Treatment Form
- Employee then supervisor completes/signs DocuSign form, and copy automatically sent to HR Benefits Counselor
