Enrollment Steps

To complete your enrollment process, you'll need to click the links below to fill out your Emergency Contact and Medical Information paperwork, as well as your Policies and Release Forms paperwork. Please note that these steps must be completed in one sitting.

See below for a list of information and materials you will need to complete both steps:

• custodial parent contact information
• primary and secondary emergency contact information 
• authorized carpoolers
• physician information 
• medical insurance card 
• date of last tetanus immunization 
• current medical conditions, medication and allergies
• roommate requests (if any)
• program-specific supplemental release forms (see below)

Supplemental Forms

We partner with top-notch organizations to help implement our surf and recreation activities. These organizations require certain forms themselves for your student to participate. Please download and complete the forms below if your student is participating in any of the programs listed. Please upload all completed supplemental forms in Step 2.
