UNCW's Registrar Office can help you handle changes to your UNCW permanent record, whether that be your legal name, gender, address, or Social Security Number.
Current and former students need to bring the documents listed below to the Office of the Registrar (James Hall) and complete the Change of Name Form. Only one ID is required if a valid government-issued photo ID is provided (ex: driver's license, passport, state ID card, etc.). Individuals presenting identification that is not a photo ID (ex: SSC, divorce decree, voter registration card, court order, etc.) will need to also present a photo ID to accompany their other form of identification.

Name Change with Notary: Please use the ‘Name Change with Notary’ section on the Change of Name form. You must bring either one valid photo ID or one other government-issued identification and a valid photo ID to show the Notary Public. Sign the Change of Name form in the presence of the Notary. Once the Notary has notarized the Name Change form, the form must be mailed, faxed, or emailed to the UNCW Office of the Registrar. Do not send scanned copies of government-issued identification, only the name change form with the completed notarization section.
Name Change with Office of the Registrar via Zoom: Please schedule a time to Zoom with a Registrar staff member by sending an email to registrar@uncw.edu. A working web camera and microphone is required. You must have either one valid photo ID or one other government-issued identification and a valid photo ID to show the Registrar staff member. After the acceptable forms of ID are verified, the staff member will watch the student sign the Name Change form. Physical or electronic signatures are acceptable. Once the form is signed and the documents are verified, the form must be mailed, faxed, or emailed to the UNCW Office of the Registrar. Do not send scanned copies of government-issued identification, only the name change form with the completed notarization section.

Bring the documents listed below to Human Resources (Administrative Annex, Rm 1045)
Any student at the University of North Carolina Wilmington can indicate a preferred first name to the university community. A legal change of name is not required for this.
The preferred name appears in many internal locations such as the online campus directory, SeaNet, mySeaport, email, grade reports, class rosters, and advisee lists. Recording a preferred name is completed in SeaNet in the Personal Information area and the Enter Preferred First Name link.
Please note: a preferred first name may be viewable to any proxy access users who view a student’s information in SeaNet.
Whether you're correcting or adding the social security number in your record, you will need to complete the Submission or Correction of Social Security Number form. This form must be completed in person, witnessed by an Office of the Registrar staff member. Additionally, you will need to show the original signed social security card along with a government-issued photo ID (ex: driver's license, passport, state ID card, etc.).

You must bring the social security card and one of which must be a government issued photo ID to show the Notary Public. Sign the Submission or Correction of SSN form in the presence of the Notary. Once the Notary has notarized the Submission or Correction of SSN form, the form must be mailed, faxed, or emailed to the UNCW Office of the Registrar.

After the acceptable forms of ID are verified, the staff member will watch the student sign the Submission or Correction of SSN form. Physical or electronic signatures are acceptable. Once the form is signed and the documents are verified, the form must be mailed, faxed, or emailed to the UNCW Office of the Registrar.
A student's official academic record reflects the gender the student identified as when he or she applied for admission to the University. Students who subsequently wish to change the gender designation in their official academic record must provide the Office of the Registrar with a certified copy of a court order showing their change of gender, or other state or federal identification, such as a revised driver's license, reflecting their new gender. "Official academic record" in this section is defined as UNCW’s student information system (BANNER).
For students who are in the process of transitioning and state or federal documentation is not yet available, a change to “Not Available” can be requested until additional documentation can be presented. However, this option is not available to employees of the university.
To request that your academic record reflect a gender change, submit a written request along with supporting documentation, to the Office of the Registrar.
Once the Office of the Registrar records a gender change in a student's official academic record, the official academic record will identify only the new gender information. Note: UNCW transcripts and diplomas do not specifically reference gender.
