Academic forms allow students to file requests with the university for things like graduation, withdrawal, and course conflicts.

Directed Individual Study Forms

Permission to Enroll in Directed Individual Study
- While students are limited to nine credit hours towards graduation, department and programs are limited to a maximum of 6 credits.
- Confirm program requirements regarding the number of accepted credits towards degree.
International Travel Directed Individual Study

Internship Forms

Permission to Enroll in an Internship
- Please note: academic credit awarded for each internship program shall not exceed SIX hours (Faculty Senate Handbook).

Enrollment/Withdrawal Forms

Undergraduate Request to Enroll in a Graduate Course
- Also, please check with the graduate coordinator for the program.
- Each program may have its own form.
CSE Over Hours Request
- Please note that the maximum number of credits that can be enrolled in is 21 credits.
- The cumulative GPA must be 3.5 or higher (UNCW Undergraduate Catalog).

CSE Withdrawal for Extenuating Circumstances
- NOTE that a request for a WE is primarily for all courses in that term.
- A "WE" does not come with a tuition refund; please work with financial aid and student accounts to understand any financial implications.
NOTE: Summer Session withdrawals
- The withdrawal passing period in summer is very brief.
- Because of the short duration of summer school sessions, summer school charges are not refundable after the add/drop period.
