Below is a table containing forms. Click on the form in the "Form" column and then SAVE the file.

OPEN THE FORM FROM THE SAVED LOCATION. THIS WILL ALLOW YOU TO COMPLETE THE FORM ELECTRONICALLY AND PROVIDE ELECTRONIC SIGNATURES.

Overview:

Traveler >

  • Traveler completes Form A
  • Traveler emails Form A to Dept. Chair, Doug Gamble
  • Form A will be returned to the Traveler by one of the following: Dept. Chair, Dean, Provost, or Chancellor.
  • If Form A is approved (Four Signatures), Email Approved Form A and Form B to Sarah Woods.
  • Upon receiving Approved Form A and Form B, Sarah will enter a Travel Pre-Approval
  • The traveler must Accept or Return the Chrome River Pre-Approval when routed for approvals

All of these steps are required PRIOR to traveling.

Plan ahead.  The approval process is lengthy.

Temporary Travel Request Approval Form

Temporary Travel Request Approval Form

Must complete prior to traveling for conferences, research, etc. If you need to know the appropriate fund number for your requested travel, send an email to Sarah Woods. Send completed form to Doug Gamble. Traveler
Approved Temporary Travel Request Approval Form (Traveler will receive from the Chancellor's Office) Approved Travel Request = Form A with all 4 required Approvals The traveler will receive an Approved or Denied Form A from the Chancellor's Office. If denial occurs prior to reaching the Chancellor's Office, Form A will be returned to the traveler from a preceding entity.  If approved, submit Form A with all approval signatures AND Form B to Sarah Woods. Traveler must email Approved Form A and Form B to Sarah Woods

Current guidance requires all essential travel requests be approved by the Supervisor, Dean/AVC, Provost/Vice Chancellor and Chancellor. Non-essential travel should not occur at this time.

EOS Poster Printing Request Poster Printing Request Submit this form as soon as you know you will need a poster with a tentative target date.  Finalized printing requests/specifications need to be submitted a minimum of seven days in advance of the desired pick up date. Please allow 48 hours from file submission to pick up time. Faculty
