Thesis and Dissertation:
- Format Approval
- DocuSign form is initiated by the student then electronically signed by the advisor

- DocuSign form initiated by the student that specifies the electronic release information for the thesis or dissertation
Withdrawal:
- Complete Withdrawal from Program
- DocuSign form initiated by student, that should be used when the student is withdrawing from the program and the university, with no plans to return.
- Course Withdrawal
**Students who withdraw from all courses during a semester should complete a Leave of Absence form to be eligible to continue with course work at a later semester.** - Leave of Absence (Regular Term Instruction-RTI programs)
