Graduate School Forms

Forms for Current Graduate Students:

Thesis and Dissertation:

  • Format Approval
    • DocuSign form is initiated by the student then electronically signed by the advisor
  • EDT Submission Form
    • ​​​​​​​DocuSign form initiated by the student that specifies the electronic release information for the thesis or dissertation

Withdrawal:

  • Complete Withdrawal from Program
    • ​​​​​​​DocuSign form initiated by student, that should be used when the student is withdrawing from the program and the university, with no plans to return.  
  • Course Withdrawal
        **Students who withdraw from all courses during a semester should complete a Leave of Absence form to be eligible to continue with course work at a later semester.**
  • Leave of Absence (Regular Term Instruction-RTI programs)
  • Leave of Absence (Online Accelerated Programs-OAP)
