Survey Tool - "Select Survey"

select survey box imageSelect Survey is a Web-based survey application available to faculty and staff for creating, deploying and analyzing professionalquestionnaires or surveys. Responses can be sent to an email address and/or saved in a database. Questions and answers are created via an easy-to-use Web interface, and data can be exported at any time.

Access to Create and Manage Surveys

Faculty and Staff can access Select Survey through the LaunchPad on mySeaport. For "Create and Manage" access please submit a service request through the ITS Service Catalog using the "Request Permission to Create and Manage Surveys" Service.

Using Select Survey

  1. To use Select Survey, access the application through the mySeaport landing page ( or or by visiting,
  2. Login with your UNCW email credentials to begin.
  3. Give the survey a title that identifies your department (e.g., "WCE Survey A").
  4. Insert questions; there are about 35+ different types of questions.
  5. When complete, the survey can be previewed before deploying.
  6. When you're finished, click the "deploy" box to see the URL of the survey.
  7. Make sure to go through the "options" to set an accurate Start/End date; the survey is automatically closed to the public on the End date.

Helpful Notes:

  • You can login at any time to analyze your data. The Analyze option will also let you export results to Excel.
  • Select Survey accounts can be created for UNCW student employees. A Service Request should be entered by a student’s supervisor or faculty member for "Create" access to be granted.
  • Questions and responses cannot be restored once surveys are deleted.
    You should make backups of your survey on your local machine.

Sensitive Information is Not Permitted

This tool cannot be used to create surveys that ask for any sensitive information.
