Programming Forms

The following forms must be filled out in their entirety and emailed to Steve Myrick at at least 5 business days prior to your program in order to be processed in time.

  • Equipment Reservation Calendar
    • To reserve equipment through the Equipment Reservation Form, send a calendar invite to Chris Livernois at
    • These forms can be used to reserve: PA systems, blow-up movie screen, karaoke machine, etc.
    • Once the request is approved you will receive an Outlook calendar appointment for both pick-up and drop-off times. You must accept both invitations for the reservation process to be completed.
    • *PLEASE NOTE*: You must have an RC/ARC/CDA present when picking up and dropping off the equipment in order to get access to the equipment room.
  • Please email Cierra Washington at to reserve RHA equipment.
    • Include your area, the program time and place, and what the equipment will be used for.
    • Equipment that can be reserved includes: tarps, bins, the camera and the popcorn machine.
    • Equipment must be cleaned thoroughly and returned within 48 hours proceeding the event.
