Immunizations and Medical Records

The Immunization & Medical History Form must be correct and complete prior to attending classes. Students will be notified of any incomplete or missing information through their UNCW email address.

North Carolina law (General Statute 130A 152-157) requires that all students entering college must present a "Certificate of Immunization" within thirty days of registration. By NC law, you will be withdrawn from the University if you do not provide documentation of the required immunizations prior to the deadline. We prefer that completed forms are received at the Student Health Center prior to orientation, or before June 30th if your orientation is later in the summer. You can mail, email, fax, or bring by your forms to the Abrons Student Health Center, 2nd Floor, DePaolo Hall.

Medical Records

Students can request their medical records in person, via mail, email or fax. Please complete the release form and mail it to the address above, email it to immunizations@uncw.edu, or fax to 910-962-4130. Allow a minimum of 48 hours from receipt for duplicating and mailing of medical records.

Medical records, including immunization records, copied for admission to another school, for continuity of care, or referrals to other health care providers are free. There will be a small charge for records requested for other reasons.

Medical records are maintained on all UNCW students – these include the information you provided when you enrolled at UNCW and any records that were generated as a result of medical care you received at the Student Health Center. Information in the medical record is confidential and access is restricted to Student Health Center staff. Your medical records will not be released to anyone unless you provide written consent or when required by public health law or by court order. Know that we take your confidentiality seriously.

Medical or Religious Exemptions

Students requesting a Medical or Religious Exemption must have those forms submitted within 30 days of registration as well. Again, we prefer that completed forms are received at the Student Health Center prior to orientation, or before June 30th if your orientation is later in the summer.
