All plans offer in-network and non-network benefits. Using an in-network provider results in less expense to the employee. Contact Superior Vision Services or your benefits counselor in HR for a list of in-network providers.
Eligibility: Permanent employees working 50% time or more are eligible to enroll in vision care insurance. Eligible dependents include spouse or unmarried children up to age 19, or if enrolled as a full-time student up to age 26. *Coverage is available to unmarried partners on an after-tax basis.
Enrollment: There is no waiting period for first-time enrollees. You can not change your election in the vision plan until the next annual enrollment period (October). However this is a two-year plan. If you elect coverage this year and drop coverage the following year, you will have to wait an additional two years before you can get back in the plan.
Changing your NCFlex Election: You will not be able to cancel or change your election until the end of the plan year unless you have a qualified family status change. Changes without a qualified family status change are not allowed in NCFlex benefits during the plan year in accordance with Section 125 of the Internal Revenue Service code. If you wish to change your elections, you must notify your HR benefits counselor of any change in status within 30 days of the event. The changes you want to make to your benefits must be consistent with the change in your status.
Examples of qualified family status changes are:
- birth or adoption of a child
- spouse becomes employed/unemployed
- you or your spouse change from part-time to full-time employment (or vice-versa)
- you or your spouse take an unpaid leave of absence
More examples of qualified family status changes can be found in the NCFlex Enrollment booklet.
