Medical Records

The SHC Medical Records Department is responsible for receiving, filing, and securing immunization records, medical records, and telephone/fax messages regarding care.  All students presenting for health care will have a medical record on file and are required to complete the

"Immunization & Medical History Form".

The Immunization & Medical History Form must be correct

and complete prior to attending classes.  Students will be notified of any incomplete information. Completed forms can be sent to the Student Health Center prior to orientation, or as early as possible. Forms can be mailed to:

Copies of Medical Records

If a student wishes to release a medical record, a release form must be signed. Allow a minimum of 48 hours for duplicating and mailing of medical records. Medical records & immunization records copied for referrals to other health care providers are free. A fee is charged for copies made for any other purpose.

North Carolina law (General Statute 130A 152-157) requires that all students entering college must present a "Certificate of Immunization" within thirty days of registration. By NC law, you will be withdrawn from the University if you do not provide documentation of the required immunizations prior to the deadline. You must provide proof that you have received the immunizations that are required by law.
