The forms below have been approved by University Counsel, Academic Council and Cabinet for use by all UNCW offices and/or departments who bring minors to campus for camps/programs or other camp-like events. The office/department responsible for the camp/program must have a set of forms completed and signed by the participant's parent or legal guardian.
