Full Cycle Performance Appraisal Submission

Before you begin...

  • The supervisor should upload performance appraisals; NOT the employee
  • Save your completed performance appraisals as PDFs
  • Make sure you know where you have saved them
  • For consistency, name your completed performance appraisals as follows:
    • Last Name + First Initial + Cycle Year
    • For Example, Sammy Seahawk's appraisal would be named: SeahawkS16-17.pdf

  • NOTE: Do NOT tab between fields when completing the form; it will reset what you've done!

How to submit a completed Full Cycle Performance Appraisal:

1. Go to the submission form (CLICK HERE)
You may be prompted to sign in; simply use your UNCW email credentials

2. Choose the cycle year (2016-2017)
3. Choose your department
The department names are the same as on UNCW's directory. Not sure about yours? CLICK HERE to go to the directory to find out. 
4. Click the "book" icon to find and select the supervisor's name
5. Do the same for the employee's name
6. Choose the employee's appropriate classification (SHRA or EHRA)
Please Note: The EHRA option refers only to EHRA non-faculty employees
7. Choose the employee's overall rating
Be sure this matches the rating on the performance appraisal!
8. Attach the completed appraisal by clicking "Click here to attach a file" and following the prompts
9. Attach any additional documents at this time
This is optional; additional documents may be employee comments or an individual goal addendum
10. Click the "Save" button in the upper left-hand corner of the screen to submit the form
A pop-up box will appear to let you know that your submission was successful; click OK
11. To upload an appraisal for another employee, click the "Add new item" button and a new form will appear. Repeat steps 1-10!
